Why your US health plan stops at the border

Most US health insurance, employer plans included, treats coverage outside the country as an afterthought at best. Some plans pay a portion of emergency care abroad; plenty pay nothing at all once you leave the US, Canada, or Mexico. Medicare is more blunt about it: outside a narrow set of exceptions near the Canadian and Mexican borders, Original Medicare doesn't cover care abroad, and most Medicare Advantage and Medigap plans that do offer any foreign travel benefit cap it at a modest lifetime or per-trip limit. A hospital in Bangkok or Lisbon has no reason to bill a US insurer directly, and plenty will ask for payment, or at least a large deposit, before treating you. Match the plan to the trip, not the other way around

The category you need depends more on the shape of your trip than on your destination. A two-week vacation calls for a comprehensive plan that bundles trip cancellation with emergency medical, since protecting the money you already spent matters as much as the medical coverage itself. A few months of remote work abroad points toward nomad insurance instead, coverage sold monthly with no fixed end date, built for someone who doesn't know exactly when they're coming back. Settling somewhere for a year or more is a different product again, closer to real international health insurance than a travel policy. Then there are the sharper edge cases: backpacker and adventure plans that name the specific activities they cover, coverage built for travelers over 65 or 70, plans that satisfy a school's insurance waiver or a J-1 visa sponsor's requirement, and visitor plans for foreign nationals coming to the US, where healthcare pricing makes insurance close to mandatory in practice.

The four things that actually decide whether a plan works for you

Skip the marketing copy and read for four specific things. The medical maximum is the total dollar amount the plan will pay out, and it needs to be high enough to cover a real hospitalization in your destination, not just a doctor's visit. The deductible is what you pay first, and a lower deductible costs more in premium but saves you from an unexpected bill later. Whether coverage is primary or secondary matters more than it sounds: primary pays directly, secondary only pays after your existing insurance (if you have any that applies) has paid its share first, which can mean a slower reimbursement process instead of direct billing at the hospital. And check who's actually paying: a comparison site or marketplace lists plans, but the insurer behind whichever one you pick is who settles your claim.

Two exclusions that catch people off guard

Pre-existing conditions and adventure sports are where more claims get denied than almost anywhere else in a policy. Most plans exclude pre-existing conditions outright unless you buy within a specific window after your first trip payment, often 14 to 21 days, and even then coverage is usually for an "unexpected recurrence" rather than ongoing treatment of a known condition. Separately, "adventure" isn't a single category insurers agree on: scuba diving past a shallow depth limit, skiing off-piste, or trekking above a stated altitude can all fall outside a standard plan's base coverage even if the activity feels routine to you. If your itinerary includes either situation, look for a plan that names the specific condition or activity rather than assuming a general policy has you covered.

When a medical plan isn't enough

A travel medical plan pays your hospital bill. It doesn't necessarily get you out of a dangerous or remote situation, that's a separate category: evacuation and rescue memberships. These aren't insurance in the usual sense, they're prepaid services that arrange transport, sometimes to a hospital of your own choosing rather than the nearest adequate one, or extraction from somewhere a standard ambulance can't reach. Frequent travelers, expedition trekkers, and anyone heading somewhere genuinely remote often carry one of these memberships alongside, not instead of, a regular medical policy.
