World Nomads started in Australia in 2002 and built its reputation on two things: an activity list that names well over a hundred specific sports and pursuits, from scuba diving to white-water rafting to trekking above a stated altitude, and the ability to buy or extend coverage while you're already mid-trip. That second feature matters more than it sounds, since most travel insurers require you to buy before departure and won't touch a policy once you're abroad.

Coverage is secondary to other insurance you hold, and pre-existing conditions are excluded unless specifically declared and accepted at purchase. The Explorer plan covers mountaineering and altitude up to roughly 7,000 meters, while the Epic plan pushes higher for genuine expedition travel, so the right tier depends on exactly what you're doing rather than a single blanket policy. It's a strong match for long, multi-country backpacking routes and one-off adventure trips where a standard travel medical plan's exclusion list would otherwise leave you exposed.
